If your name and passport number are changed, you shall fill the FORM FOR INFORMATION CHANGE OF OVERSEAS TEACHERS (downloaded from the website of the Office of International Cooperation and Exchange), then apply to the Office of International Cooperation and Exchange for the information changes of work permit and residence permit within 30 working days from the date of your information change.
